When the heart is being compressed by fluid in the pericardial sac, the immediate problem is impaired filling and reduced cardiac output. The fastest way to reverse this is to decompress the pericardium, which is exactly what pericardiocentesis does. By draining the blood from the pericardial space, it quickly relieves the pressure on the heart, restores venous return, and improves stroke volume and blood pressure. This rapid stabilization is why it’s the primary management for traumatic tamponade.

Observation offers no relief of the pressure and can allow continued collapse. Surgical repair without drainage won’t address the tamponade itself and leaves the heart constricted. Chest compressions don’t relieve the pericardial constraint and can cause additional injury without resolving the underlying problem.
